Narrative:
Aircraft X has a recurring door squeal coming from the L-1 door. This problem has been occurring for over 2 months. I was assigned to investigate it. I determined the leak was coming from the external handle area. Removal of the door liner was necessary to find the leak's source, and the first step of door liner removal is to remove the door evacuate/evacuation slide. When it was time to reattach the door slide, I notified the on-shift quality control inspector to sign off the slide installation. The aircraft was parked at ZZZ cargo facility, while the aircraft logbook was in the line maintenance office. I documented the slide removal and reinstallation on technician's non-routine write-up which is attached to the service check paperwork which I did have at the aircraft. Inspector stamped on the rii buyback block of the technician's non-routine write-up. I did reference the technician's non-routine write-up number on the maintenance history computer entry, but failed to reference the inspector's employee number.

Title: AN MD80 DOOR ESCAPE SLIDE WAS REMOVED TO REPAIR A DOOR LEAK. SLIDE WAS REINSTALLED AND REQUIRED INSPECTION COMPLETED. INSPECTOR'S EMPLOYEE NUMBER NOT ENTERED IN COMPLETED PAPERWORK.
