Narrative:
On arrival into lax while the captain was the PF, he failed to comply with the ATC clearance for the pdz 4 arrival and also via radar vectors. The captain failed to comply with the 17000 ft restr at konzl by leveling off at 16600 ft after he was told several times to level off by myself and the flight engineer. The captain failed to comply with the trtle restr of crossing between 16000-17000 ft. He leveled off at 15700 ft again after myself and flight engineer told to level off at 16000 ft. The captain failed to comply with the speed restr assigned by socal approach 5 times -- once by 20 KTS, 4 times by 10 KTS. He was informed by the crew to maintain his assigned speed in all of the incidents. The captain's poor airmanship, lack of CRM and lack of understanding of STAR charts were the causes of the above incidents. Retraining of this captain is the only way to correct for the above errors. Callback conversation with reporter revealed the following information: the reporter stated that he believes the captain's lack of familiarity with stars contributed to the altitude crossing non compliance. The reporter said that the captain was not defiant, just did not respond because he believed that he was cleared to descend below the crossing constraints. The captain simply ignored the first officer's and flight engineer's leveloff calls. After the second missed constraint, approach control vectored the flight off of the arrival. This analyst asked about professional standards intervention and the reporter stated that he did contact professional standards.

Title: THE CAPT OF A DC10-30 TWICE DSNDED BELOW ALT CONSTRAINTS ON A STAR. HE WOULD NOT CORRECT THE DEV IN RESPONSE TO EITHER THE FO'S OR FE'S ALTDEV CALLOUTS.
Narrative: ON ARR INTO LAX WHILE THE CAPT WAS THE PF, HE FAILED TO COMPLY WITH THE ATC CLRNC FOR THE PDZ 4 ARR AND ALSO VIA RADAR VECTORS. THE CAPT FAILED TO COMPLY WITH THE 17000 FT RESTR AT KONZL BY LEVELING OFF AT 16600 FT AFTER HE WAS TOLD SEVERAL TIMES TO LEVEL OFF BY MYSELF AND THE FE. THE CAPT FAILED TO COMPLY WITH THE TRTLE RESTR OF XING BTWN 16000-17000 FT. HE LEVELED OFF AT 15700 FT AGAIN AFTER MYSELF AND FE TOLD TO LEVEL OFF AT 16000 FT. THE CAPT FAILED TO COMPLY WITH THE SPD RESTR ASSIGNED BY SOCAL APCH 5 TIMES -- ONCE BY 20 KTS, 4 TIMES BY 10 KTS. HE WAS INFORMED BY THE CREW TO MAINTAIN HIS ASSIGNED SPD IN ALL OF THE INCIDENTS. THE RPTR SAID THAT THE CAPT WAS NOT DEFIANT, JUST DID NOT RESPOND BECAUSE HE BELIEVED THAT HE WAS CLRED TO DSND BELOW THE XING CONSTRAINTS. THE CAPT SIMPLY IGNORED THE FO'S AND FE'S LEVELOFF CALLS. AFTER THE SECOND MISSED CONSTRAINT, APCH CTL VECTORED THE FLT OFF OF THE ARR. THIS ANALYST ASKED ABOUT PROFESSIONAL STANDARDS INTERVENTION AND THE RPTR STATED THAT HE DID CONTACT PROFESSIONAL STANDARDS.
