Narrative:
Simultaneous departures runway 18L&right. Runway 18L SID is to turn left to 165 degree heading. Runway 18R is straight out. On runway 18R; air carrier Y came off on wrong beacon. On runway 18L; air carrier X came off without flight progress strip. We were having printer trouble. I thought I gave runway 18L to air carrier 80 but no strip marking due to no strip. No response. Issued turn to 210 degrees to air carrier Z to join bluegrass departure SID. Air carrier west was 30 mi southwest and I issued climb and turn to join SID. No response. Issued to air carrier west again and I got a bad readback. Restated instructions to air carrier west. At some point; air carrier xx off runway 18R acquired. Air carrier Y went through SID and was converging; although out in front of air carrier xx. Issued 240 degree turn to air carrier xx to join SID. Turned air carrier X back southeast toward SID. Factors: 1) bad beacon code runway 18R departure. 2) no progress strip runway 18L departure and subsequent no strip marking as memory jogger for me to verify 60 or 80 issued. 3) runway 18L air carrier X departure went through the SID toward runway 18R departure. 4) missed transmission and readback of air carrier west; 30 mi southwest; was time consuming. 5) 6TH day of my work week.

Narrative: SIMULTANEOUS DEPS RWY 18L&R. RWY 18L SID IS TO TURN L TO 165 DEG HDG. RWY 18R IS STRAIGHT OUT. ON RWY 18R; ACR Y CAME OFF ON WRONG BEACON.
