Narrative:

On the day of the incident; at the beginning of my shift; I received my work package. In my work package; I had paper for a #1 check; and later the next morning when I signed the cover sheet; it read a #2 check; so I lined through the number '2' and put number '1.' later I was told that it was an error in the paperwork; it was supposed to have had a service check but it had been downgraded to a #1 check. In the future; I feel that the supervisor should make sure that all cover sheets and paperwork are the same.
