Narrative:
Taxi out to runway 26L ont. While captain on PA; tower gave cleared into position and hold. Captain gave thumbs up that he'd heard and I read back clearance as we went into position. I said 'shall I take it on the roll?' captain said 'yes' and away we went without clearance for takeoff. Captain got clearance on the roll. Not good. Ont tower very accommodating; though. Factors: lack of sleep; multi-tasking; distrs in cockpit (no van at pick-up making us late to jet); task saturation that I either did not see coming or didn't want to admit. Supplemental information from acn 672383: XA30 wake-up. Should have been extremely vigilant after not having flown since september. They were ready to push back from their ground crew perspective before I was ready and briefed in the cockpit. We accomplished appropriate checklists; and pushed back 7 mins early.

FACTORS: LACK OF SLEEP; MULTI-TASKING; DISTRS IN COCKPIT (NO VAN AT PICK-UP MAKING US LATE TO JET); TASK SATURATION THAT I EITHER DID NOT SEE COMING OR DIDN'T WANT TO ADMIT. SUPPLEMENTAL INFO FROM ACN 672383: XA30 WAKE-UP. SHOULD HAVE BEEN EXTREMELY VIGILANT AFTER NOT HAVING FLOWN SINCE SEPTEMBER. THEY WERE READY TO PUSH BACK FROM THEIR GND CREW PERSPECTIVE BEFORE I WAS READY AND BRIEFED IN THE COCKPIT. WE ACCOMPLISHED APPROPRIATE CHKLISTS; AND PUSHED BACK 7 MINS EARLY.
