Narrative:
Air carrier X was cleared for takeoff. Another aircraft was taxied into position and hold. The second aircraft said he was not ready. The controller said to cancel takeoff clearance and forgot about the first air carrier X taking off. Then the controller crossed air carrier Y in front of the first air carrier X aircraft.

Title: LAS TWR SUPVR DESCRIBES CONFLICT ON RWY AS LCL CTLR CROSSED TFC IN FRONT OF DEP ACFT.
Narrative: ACR X WAS CLRED FOR TKOF. ANOTHER ACFT WAS TAXIED INTO POS AND HOLD. THE SECOND ACFT SAID HE WAS NOT READY. THE CTLR SAID TO CANCEL TKOF CLRNC AND FORGOT ABOUT THE FIRST ACR X TAKING OFF. THEN THE CTLR CROSSED ACR Y IN FRONT OF THE FIRST ACR X ACFT.
