Narrative:

E120 was told to cross runway 25R on taxiway F. As the E120 was entering the runway; the SF34 was cleared for takeoff. The assistant (local assist #1) told the controller (local controller #1); 'you're crossing.' his response; 'what?' assist told him; 'cancel that clearance; you're crossing at taxiway F.' just then one of the pilots questioned the order. The clearance was never canceled. He told the departure; 'traffic was crossing at taxiway F' and told the crossing to continue to cross. The assist waited about 5 mins to give the controller time to deal with traffic and advise the supervisor. When he didn't; the assist advised the controller; 'you need to tell mr X about air carrier X and air carrier Y before they call.' the controller responded 'ok; but I really don't think they'll call.' 1/2 hour later and still no report to management; the assist again advised the controller to report it. The controller responded; 'I will.' when the assist was relieved; the situation was reported to mr X (the supervisor) via telephone. At the end of the shift I was advised the controller never advised management.

Title: LAX TWR ASSISTANT LCL CTLR DESCRIBED LOSS OF SEPARATION AS TAXIING TFC WAS CLRED TO CROSS RWY DOWNFIELD FROM DEP TFC; NOT RECORDED BY FAA AS ANY TYPE OF INCIDENT.

Narrative: E120 WAS TOLD TO CROSS RWY 25R ON TXWY F. AS THE E120 WAS ENTERING THE RWY; THE SF34 WAS CLRED FOR TKOF. THE ASSISTANT (LCL ASSIST #1) TOLD THE CTLR (LCL CTLR #1); 'YOU'RE XING.' HIS RESPONSE; 'WHAT?' ASSIST TOLD HIM; 'CANCEL THAT CLRNC; YOU'RE XING AT TXWY F.' JUST THEN ONE OF THE PLTS QUESTIONED THE ORDER. THE CLRNC WAS NEVER CANCELED. HE TOLD THE DEP; 'TFC WAS XING AT TXWY F' AND TOLD THE XING TO CONTINUE TO CROSS. THE ASSIST WAITED ABOUT 5 MINS TO GIVE THE CTLR TIME TO DEAL WITH TFC AND ADVISE THE SUPVR. WHEN HE DIDN'T; THE ASSIST ADVISED THE CTLR; 'YOU NEED TO TELL MR X ABOUT ACR X AND ACR Y BEFORE THEY CALL.' THE CTLR RESPONDED 'OK; BUT I REALLY DON'T THINK THEY'LL CALL.' 1/2 HR LATER AND STILL NO RPT TO MGMNT; THE ASSIST AGAIN ADVISED THE CTLR TO RPT IT. THE CTLR RESPONDED; 'I WILL.' WHEN THE ASSIST WAS RELIEVED; THE SITUATION WAS RPTED TO MR X (THE SUPVR) VIA TELEPHONE. AT THE END OF THE SHIFT I WAS ADVISED THE CTLR NEVER ADVISED MGMNT.
