Narrative:

I noticed traffic at approximately my 9 O'clock position. It appeared he was near our altitude and coming at us. I then checked the TCAS just as he was coming on the screen and noted he was in a descent; 800 ft above us. I told the first officer to call center and ask about traffic and center hollered at him. He was supposed to be at FL340. I told the first officer to prepare for a possible conflict. Immediately after I spoke; we got a traffic alert and we immediately got 'climb; climb.' I performed the avoidance maneuver and as best I could tell; we missed him by no more than 100-200 ft as indicated by the TCAS presentation.
