Narrative:
Rejected takeoff. Aircraft deiced at gate and again at a car wash location in clear WX. With engine anti-ice on after a 15 second engine run-up applied maximum power setting. Experienced compressor stall on engine #3; rejected takeoff. Upon postflt; ice was discovered in the inlet of engine #1. Suspect same on #3. Maintenance discovered inlet fan damage on #3. Poor deice crew training/performance to blame. Pilot fatigue: air carrier X feb/sun/06 showed at XA50. Push approximately at XB20. Car wash deice. Rejected takeoff; block back in at XC30. Airplane hand down from ice damage to engine. Waited for dispatch to decide what to do. Dispatch stated that they were overwhelmed and unable to find time to address problem. XH00 decision made to use new airplane. XH55 ground crew begins unloading first airplane. XJ00 ground crew finishes loading current plane. XJ45 ground crew return with inaccurate weight and balance paperwork. XK00 no paperwork yet available. Crew (captain first officer) forced to call in fatigue. So did not call in fatigue as he was concerned because he was on first yr probation. 10:30 duty. 5:00 sitting in cockpit. 5:00 waiting in flight operations. Company (scheduler; dispatch; maintenance; ground crew) unprepared to fly after winter WX. Poor deice training. Lack of dispatch; scheduling staffing led to inability to operate airline. Duty officer was concerned only for legality not safety. FAA should require different duty limits for different operations.
