Narrative:

On departure from teb on the teb 5 departure runway 24; is assigned to climb to runway heading until 1500 ft; then right turn to a heading of 280 degrees until 4.5 DME from teb VOR then climb to 2000 ft. Failure of the crew to comply with the 4.5 mi from the teb VOR to execute the climb from 1500 ft to 2000 ft; until we were transferred from teb tower to newark departure and notified by this error.

Narrative: ON DEP FROM TEB ON THE TEB 5 DEP RWY 24; IS ASSIGNED TO CLB TO RWY HEADING UNTIL 1500 FT; THEN R TURN TO A HEADING OF 280 DEGS UNTIL 4.5 DME FROM TEB VOR THEN CLB TO 2000 FT.
