Narrative:
Under pressure to fulfill air carrier X request; combined with the approaching departure deadline; mrs X made a bad decision. She asked the fueler to rechk the fuel density and used only 1 fuel hose hook-up. She disabled the volumetric top-off system and pulled the fuel overfill control circuit breaker and told the fueler to add carefully 1500 kilos of fuel to the center tank only. Before the fueler could complete his load; fuel started to spill out on both wingtips.

Title: A B747-400 WHILE BEING FUELED EXPERIENCED A LARGE FUEL SPILL DUE TO OVERFUELING CAUSED BY FAILURE TO FOLLOW FUELING PROCS.
Narrative: UNDER PRESSURE TO FULFILL ACR X REQUEST; COMBINED WITH THE APCHING DEP DEADLINE; MRS X MADE A BAD DECISION. SHE ASKED THE FUELER TO RECHK THE FUEL DENSITY AND USED ONLY 1 FUEL HOSE HOOK-UP. SHE DISABLED THE VOLUMETRIC TOP-OFF SYS AND PULLED THE FUEL OVERFILL CTL CIRCUIT BREAKER AND TOLD THE FUELER TO ADD CAREFULLY 1500 KILOS OF FUEL TO THE CTR TANK ONLY. BEFORE THE FUELER COULD COMPLETE HIS LOAD; FUEL STARTED TO SPILL OUT ON BOTH WINGTIPS.
