Narrative:
After the audit I was made aware that there were several engineering orders accomplished and the paperwork for these were not completed. After this was brought up to me; I do recall this day. We were assigned to perform these tasks without the paperwork generated. In order to expedite the operation; we were instructed that we needed to complete this task without the paperwork. These discrepancies were affecting the operation as a whole; due to that all aircraft cargo doors #4 were not locking properly. This was a hot item. Therefore; causing chaos in the operation. We were instructed at that time by the vp of maintenance to provide repair (solution) to this locking device and that an engineering order was going to be generated accordingly to our repair. It took several weeks to complete this task on the aircraft affected. During this time period an engineering order was issued while we were repairing the aircraft; however; it had discrepancies and required to be re-done. This job was not being documented. We finished doing the fleet; time passed and both parties forgot about our responsibilities. Somewhere in the mid of last year; the engineering order was generated correctly and we forgot to complete the same.

Title: ACR MECH COMPLETES A TASK WITHOUT THE APPROPRIATE PAPERWORK AND DOCUMENTATION FROM THE COMPANY.
Narrative: AFTER THE AUDIT I WAS MADE AWARE THAT THERE WERE SEVERAL ENGINEERING ORDERS ACCOMPLISHED AND THE PAPERWORK FOR THESE WERE NOT COMPLETED. AFTER THIS WAS BROUGHT UP TO ME; I DO RECALL THIS DAY. WE WERE ASSIGNED TO PERFORM THESE TASKS WITHOUT THE PAPERWORK GENERATED. IN ORDER TO EXPEDITE THE OP; WE WERE INSTRUCTED THAT WE NEEDED TO COMPLETE THIS TASK WITHOUT THE PAPERWORK. THESE DISCREPANCIES WERE AFFECTING THE OP AS A WHOLE; DUE TO THAT ALL ACFT CARGO DOORS #4 WERE NOT LOCKING PROPERLY. THIS WAS A HOT ITEM. THEREFORE; CAUSING CHAOS IN THE OP. SOMEWHERE IN THE MID OF LAST YEAR; THE ENGINEERING ORDER WAS GENERATED CORRECTLY AND WE FORGOT TO COMPLETE THE SAME.
