Narrative:

I was notified that personal breathing equipment part number and serial number had an incorrect 'a' code based on its date of manufacture. System showed A17 (10 yr discard) but it should have been A1322 (5 yr discard). Based on a manufacture date of dec/2000; this personal breathing equipment should have been discarded in dec/2005. After confirming this information; I discussed the problem and submitted a special request to planning department to have the personal breathing equipment replaced. The special request was entered by planning.
