Narrative:

I do realize; as the PIC; I should have caught the error before flying into rvsm airspace on jul/mon/06 and jul/tue/06. This act was unintentional and will not happen again. As of today; 1 day after the incident; we had the point of contact changed out of our former chief pilot's name for the MEL with the FSDO; and we have formally requested the renewal of the letter of authority/authorized for rvsm with the FSDO as well. This situation has taught us a very valuable lesson about the importance of clarifying paperwork procedures and always doublechking the work of a fellow pilot; even at the risk of it incurring conflict within a flight department.
