Narrative:
Departing iad; failed to obtain pre departure clearance. Departure asked to squawk XXXX; looked for pre departure clearance and could not find it. Pre departure clearance obtained via ACARS. Missed pre departure clearance during route check and transponder on checklist. Often departing islands there is no pre departure clearance to verify with route. The respond on transponder is TA/RA. Having a checklist response of 'set' a crew would be more likely to check that the transponder is set to TA/RA and the correct code is set as well. Will be more careful in checking pre departure clearance.
Original NASA ASRS Text
Title: A B757-200 CREW FAILED TO REQUEST A PDC PRIOR TO DEP AND TOOK OFF WITHOUT THE CORRECT XPONDER CODE.
Narrative: DEPARTING IAD; FAILED TO OBTAIN PDC. DEP ASKED TO SQUAWK XXXX; LOOKED FOR PDC AND COULD NOT FIND IT. PDC OBTAINED VIA ACARS. MISSED PDC DURING RTE CHK AND XPONDER ON CHKLIST. OFTEN DEPARTING ISLANDS THERE IS NO PDC TO VERIFY WITH RTE. THE RESPOND ON XPONDER IS TA/RA. HAVING A CHKLIST RESPONSE OF 'SET' A CREW WOULD BE MORE LIKELY TO CHK THAT THE XPONDER IS SET TO TA/RA AND THE CORRECT CODE IS SET AS WELL. WILL BE MORE CAREFUL IN CHKING PDC.
