Narrative:

At departure load planning requested rows 20-26 be blocked due to being tail heavy. Just 100 mi prior to landing load planning called and stated that 2 pallets; one 8400 pounds and one 1515 pounds; were put on the aircraft and not accounted for. We were now nose heavy. I ACARS the flight and then made a radio call to confirm the information was received. We were close to landing and the agreement was not to land until the issue was resolved. Load planning was able to adjust the manifest and make it legal. The tail heavy departure and the nose heavy landing balanced. There is a major miscom on what is actually loaded on the aircraft. The captain called after landing. I patched load planning; the captain and myself and discussed the situation. Supplemental information from acn 715990: dispatch sent us a 'new' final weight manifest which showed an increase ZFW of about 5100 pounds! So; we took off using a 296000 pound card when in fact we were about 300000 pounds; which wasn't a critical factor; the nose heavy center of gravity was the most important factor. The first officer noted the nose felt heavy on takeoff and landing; the trim on landing with flaps 30 degrees was 10 units nose up. This is a serious problem and we have seen this happen before! I recommend a thorough investigation and retraining were required include increased supervision.

Title: B767-300 DEPARTS WITH INCORRECT WT AND BAL DATA.

Narrative: AT DEP LOAD PLANNING REQUESTED ROWS 20-26 BE BLOCKED DUE TO BEING TAIL HVY.
