Narrative:
I was working ILS approach to runway 27R at ord with simultaneous ILS runway 27L approachs with monitors. I cleared air carrier X for approach 23 mi from ord. When I said that he would intercept inside 18 mi; I told him to descend now to 3000 ft and advised him that he was still cleared for approach. In my estimation; he was going to intercept the localizer 15 mi from the airport when I was relieved. The relieving controller descended him to 2500 ft and shortly thereafter the supervisor told him to pull the aircraft out. Upon review of the tapes and radar data; this aircraft got within 2.0 mi and 900 ft of an aircraft on the other final before he was pulled out. One issue that affected this incident was that the next aircraft on final was air carrier Y which came in over the same arrival fix as air carrier X. These 2 aircraft took each other's call signs at least once.

Title: C90 CTLR EXPERIENCED OPERROR AT 2800 FT WHEN PARALLEL APCH TFC TO RWYS 27R AND 27L CONFLICTED.
Narrative: I WAS WORKING ILS APCH TO RWY 27R AT ORD WITH SIMULTANEOUS ILS RWY 27L APCHS WITH MONITORS. I CLRED ACR X FOR APCH 23 MI FROM ORD. WHEN I SAID THAT HE WOULD INTERCEPT INSIDE 18 MI; I TOLD HIM TO DSND NOW TO 3000 FT AND ADVISED HIM THAT HE WAS STILL CLRED FOR APCH. IN MY ESTIMATION; HE WAS GOING TO INTERCEPT THE LOC 15 MI FROM THE ARPT WHEN I WAS RELIEVED. THE RELIEVING CTLR DSNDED HIM TO 2500 FT AND SHORTLY THEREAFTER THE SUPVR TOLD HIM TO PULL THE ACFT OUT. UPON REVIEW OF THE TAPES AND RADAR DATA; THIS ACFT GOT WITHIN 2.0 MI AND 900 FT OF AN ACFT ON THE OTHER FINAL BEFORE HE WAS PULLED OUT. ONE ISSUE THAT AFFECTED THIS INCIDENT WAS THAT THE NEXT ACFT ON FINAL WAS ACR Y WHICH CAME IN OVER THE SAME ARR FIX AS ACR X. THESE 2 ACFT TOOK EACH OTHER'S CALL SIGNS AT LEAST ONCE.
