Narrative:
Did not fill out proper paperwork for fuel tank entry; fuel tank entry permit. Became more familiar with air carrier fueling and tank entry procedures and air carrier safety manual. Management was in such a hurry to get the work completed that the proper paperwork was not given to me.

Title: AN MD80 FUEL TANK ENTRY AND REPAIR WAS ACCOMPLISHED WITHOUT REQUIRED ENTRY PERMIT; TANK ENTRY MANUAL AND SAFETY PROCS.
Narrative: DID NOT FILL OUT PROPER PAPERWORK FOR FUEL TANK ENTRY; FUEL TANK ENTRY PERMIT. BECAME MORE FAMILIAR WITH ACR FUELING AND TANK ENTRY PROCS AND ACR SAFETY MANUAL. MGMNT WAS IN SUCH A HURRY TO GET THE WORK COMPLETED THAT THE PROPER PAPERWORK WAS NOT GIVEN TO ME.
