Narrative:
During approach to lit at 5000 ft; I noticed the cabin altitude was 4500 ft. I had the first officer ask for delaying vectors and to run the pressurization automatic system failure abnormal checklist. During the checklist we noticed that the pressurization information was gone (dashed) from EICAS. We ran the checklist to the end of the procedure line. After the checklist; the flight attendant called us to say that she was in pain from the pressure changes; the first officer and myself experienced similar discomfort. I then requested that the paramedics meet the plane at the gate. After deplaning; I called dispatch; maintenance; and my chief pilot. I went to check on the flight attendant and passenger. 1 passenger and the flight attendant asked to be transported to the hospital. None of the other passenger were treated that I saw. The aircraft had a similar write-up in the logbook. The corrective action was to replace cpam. Prior to landing we made sure that the flight attendant was able to perform her duties. She said she was.
