Narrative:

It was my leg. Took off with departure flaps 15 degrees. Briefed flaps 5 degrees in the turn; we wanted a tighter turn; due to the turn restrs. There have been a number of noise violations to the north of that SID and we wanted to be sure to stay on course. The first leveloff is 3000 ft with a sharp turn (170 degrees) to the left at 3 mi off the ILS DME. At 400 ft LNAV was engaged. At 1000 ft VNAV; climb power; flaps 5 degrees; autoplt engaged. Everything looked good; we were all hooked up and I was hands off; but it did not appear that the aircraft was going to level off at 3000 ft. We had 4000 ft in the window as that was the SID restr. 3000 ft was in the box (the FMS which is normally very reliable). Altitude was captured on the ADI and at 2500 ft I specifically remember seeing that and verbally calling that out. Just prior to 3000 ft; I disconnected the autoplt to level off. Leveled off at 3200 ft. The relief pilot thought he saw 3300 ft for the maximum altitude. Turned left for the departure and descended to 3000 ft. I probably turned off the autothrottles too because after I rolled wings level they were off. Rolled out on course and on the SID. At this point the MCP went blank; all lights off; then came back on. When it did VNAV; flight level change; and altitude hold were all on at the same time. I could not get the autothrottles to come back on despite hitting all the buttons to make them come on (speed; flight level change; VNAV; etc). At this point the relief pilot stated our flaps were still at flaps 5 degrees. Our speed was 250 KTS. With all the confusion we forgot to raise them. We raised the flaps. I think the MCP went blank again then came on again. It was a little confusing because departure was calling for us to climb to 4000 ft and we were struggling with the MCP. I did not climb right away because of the confusion and departure asked us if we had a problem. I finally just climbed with raw data as the flight directors were not working. Passing 4000-5000 ft; we got the MCP to work properly. Wrote up the flap overspeed and the MCP even though it did work properly for the rest of the flight.

Title: A B757 FLT CREW FAILED TO CAPTURE ALT ON SID. THE MCP WENT BLANK AFTER AUTOPLT SELECTED OFF AND A MANUAL RETURN TO ALT WAS COMPLETED. DURING THE DISTRACTION THE FLT CREW FAILED TO RAISE THE FLAPS FROM 5 DEGS ALLOWING THE AIRSPEED TO REACH 250 KTS.

Narrative: IT WAS MY LEG. TOOK OFF WITH DEP FLAPS 15 DEGS. BRIEFED FLAPS 5 DEGS IN THE TURN; WE WANTED A TIGHTER TURN; DUE TO THE TURN RESTRS. THERE HAVE BEEN A NUMBER OF NOISE VIOLATIONS TO THE N OF THAT SID AND WE WANTED TO BE SURE TO STAY ON COURSE. THE FIRST LEVELOFF IS 3000 FT WITH A SHARP TURN (170 DEGS) TO THE L AT 3 MI OFF THE ILS DME. AT 400 FT LNAV WAS ENGAGED. AT 1000 FT VNAV; CLB PWR; FLAPS 5 DEGS; AUTOPLT ENGAGED. EVERYTHING LOOKED GOOD; WE WERE ALL HOOKED UP AND I WAS HANDS OFF; BUT IT DID NOT APPEAR THAT THE ACFT WAS GOING TO LEVEL OFF AT 3000 FT. WE HAD 4000 FT IN THE WINDOW AS THAT WAS THE SID RESTR. 3000 FT WAS IN THE BOX (THE FMS WHICH IS NORMALLY VERY RELIABLE). ALT WAS CAPTURED ON THE ADI AND AT 2500 FT I SPECIFICALLY REMEMBER SEEING THAT AND VERBALLY CALLING THAT OUT. JUST PRIOR TO 3000 FT; I DISCONNECTED THE AUTOPLT TO LEVEL OFF. LEVELED OFF AT 3200 FT. THE RELIEF PLT THOUGHT HE SAW 3300 FT FOR THE MAX ALT. TURNED L FOR THE DEP AND DSNDED TO 3000 FT. WHEN IT DID VNAV; FLT LEVEL CHANGE; AND ALT HOLD WERE ALL ON AT THE SAME TIME. I COULD NOT GET THE AUTOTHROTTLES TO COME BACK ON DESPITE HITTING ALL THE BUTTONS TO MAKE THEM COME ON (SPD; FLT LEVEL CHANGE; VNAV; ETC). AT THIS POINT THE RELIEF PLT STATED OUR FLAPS WERE STILL AT FLAPS 5 DEGS. OUR SPD WAS 250 KTS. WITH ALL THE CONFUSION WE FORGOT TO RAISE THEM. WE RAISED THE FLAPS. I THINK THE MCP WENT BLANK AGAIN THEN CAME ON AGAIN. IT WAS A LITTLE CONFUSING BECAUSE DEP WAS CALLING FOR US TO CLB TO 4000 FT AND WE WERE STRUGGLING WITH THE MCP. I DID NOT CLB RIGHT AWAY BECAUSE OF THE CONFUSION AND DEP ASKED US IF WE HAD A PROB. I FINALLY JUST CLBED WITH RAW DATA AS THE FLT DIRECTORS WERE NOT WORKING. PASSING 4000-5000 FT; WE GOT THE MCP TO WORK PROPERLY. WROTE UP THE FLAP OVERSPEED AND THE MCP EVEN THOUGH IT DID WORK PROPERLY FOR THE REST OF THE FLT.
