Primary Problem | Maintenance Human Performance |
Narrative:
Aircraft operating unsafe. Single source of power while under ETOPS. Loss of #2 engine idg (4 times/4 different idg's) (history). In-flight shutdown of APU (history) in the united states and operating under an international leasing agreement. APU inoperative due to internal failure of metal parts/massive debris on master chip detector (scavenge pump suspected). New APU install received from ZZZZ (APU automatic shutdown in-flight). Currently flying back from ZZZZ1 on single source power. Aircraft rated ETOPS but has been downgraded. Serious electrical and flight display degradation reported by flight crew. Recently released from 'C' check with concerns and issues not addressed. Factor affecting human performance due to delayed release of aircraft from 'C' check -- aircraft was late beginning the leasing agreement. Until aircraft has a complete chkout from an FAA authority/authorized mro; this aircraft should not be flying. This aircraft has had numerous electrical and engine related issues; which were not addressed at 'C' check. This aircraft has also had prior issues of failures before 'C' check. Callback conversation with reporter revealed the following information: reporter stated the issue is the lack of adequate and proper maintenance that is not being performed on this aircraft as agreed to in the lease arrangement. This aircraft carries revenue paying passengers. This aircraft was recently released from a domestic mro maintenance provider; after an extended 'C' check. Reporter stated that as a result of the in-flight multiple engine idg electrical failures; APU failures; FMC failures; navigation data issues that have occurred; and just recently; a massive hydraulic failure on takeoff; since the aircraft was released from 'C' check; this aircraft is unsafe to fly.

Title: A MAINT SUPERVISOR REPORTS A B757-200; CURRENTLY OPERATING UNDER AN INTERNATIONAL LEASING ARRANGEMENT; WITH MULTIPLE ELECTRICAL; HYDRAULIC; NAV AND FMC DISPLAY FAILURES; IS UNSAFE TO FLY UNDER AN ETOPS ENVIRONMENT.
Narrative: ACFT OPERATING UNSAFE. SINGLE SOURCE OF PWR WHILE UNDER ETOPS. LOSS OF #2 ENG IDG (4 TIMES/4 DIFFERENT IDG'S) (HISTORY). INFLT SHUTDOWN OF APU (HISTORY) IN THE UNITED STATES AND OPERATING UNDER AN INTL LEASING AGREEMENT. APU INOP DUE TO INTERNAL FAILURE OF METAL PARTS/MASSIVE DEBRIS ON MASTER CHIP DETECTOR (SCAVENGE PUMP SUSPECTED). NEW APU INSTALL RECEIVED FROM ZZZZ (APU AUTO SHUTDOWN INFLT). CURRENTLY FLYING BACK FROM ZZZZ1 ON SINGLE SOURCE PWR. ACFT RATED ETOPS BUT HAS BEEN DOWNGRADED. SERIOUS ELECTRICAL AND FLT DISPLAY DEGRADATION RPTED BY FLT CREW. RECENTLY RELEASED FROM 'C' CHK WITH CONCERNS AND ISSUES NOT ADDRESSED. FACTOR AFFECTING HUMAN PERFORMANCE DUE TO DELAYED RELEASE OF ACFT FROM 'C' CHK -- ACFT WAS LATE BEGINNING THE LEASING AGREEMENT. UNTIL ACFT HAS A COMPLETE CHKOUT FROM AN FAA AUTH MRO; THIS ACFT SHOULD NOT BE FLYING. THIS ACFT HAS HAD NUMEROUS ELECTRICAL AND ENG RELATED ISSUES; WHICH WERE NOT ADDRESSED AT 'C' CHK. THIS ACFT HAS ALSO HAD PRIOR ISSUES OF FAILURES BEFORE 'C' CHK. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: REPORTER STATED THE ISSUE IS THE LACK OF ADEQUATE AND PROPER MAINT THAT IS NOT BEING PERFORMED ON THIS ACFT AS AGREED TO IN THE LEASE ARRANGEMENT. THIS ACFT CARRIES REVENUE PAYING PASSENGERS. THIS ACFT WAS RECENTLY RELEASED FROM A DOMESTIC MRO MAINT PROVIDER; AFTER AN EXTENDED 'C' CHECK. REPORTER STATED THAT AS A RESULT OF THE IN-FLIGHT MULTIPLE ENGINE IDG ELECTRICAL FAILURES; APU FAILURES; FMC FAILURES; NAV DATA ISSUES THAT HAVE OCCURRED; AND JUST RECENTLY; A MASSIVE HYDRAULIC FAILURE ON TAKEOFF; SINCE THE ACFT WAS RELEASED FROM 'C' CHECK; THIS ACFT IS UNSAFE TO FLY.
