Narrative:
Day 1 mr X jnu AFSS recorded the following entry: jnu ATCT informed rlls first half OTS. Soc ntyfd. 'East' entry was added by the supervisor to note equipment failure on day 2. 'Q' entry should have been added and event; because of the event was reported at the same time of tower closure. Since no 'Q' entry was entered; quality assurance did not perform a qar. Tapes were not reviewed to see when approach failed or if jnu tower advised anchorage center of the outage at cob. Day 10 talking to mr X he informed me that the controller did not contact soc. NOTAM was not issued until after the tower closure. When the approach lights are out of service in jnu; landing requirements change. Anchorage center; IFR pilots inbound to jnu and air carrier operations all need this information as soon as possible. Pilots may need to divert to an alternate airport. Summery -- violation of 7110.65. Non-compliance of letter of agreement between anchorage center and jnu AFSS. Jnu tower has a history of handing their duties to jnu AFSS at tower closure. (Hold for release clrncs; personal and equipment and equipment outages.)

Narrative: DAY 1 MR X JNU AFSS RECORDED THE FOLLOWING ENTRY: JNU ATCT INFORMED RLLS FIRST HALF OTS. SOC NTYFD. 'E' ENTRY WAS ADDED BY THE SUPVR TO NOTE EQUIPMENT FAILURE ON DAY 2. 'Q' ENTRY SHOULD HAVE BEEN ADDED AND EVENT; BECAUSE OF THE EVENT WAS RPTED AT THE SAME TIME OF TOWER CLOSURE. SINCE NO 'Q' ENTRY WAS ENTERED; QUALITY ASSURANCE DID NOT PERFORM A QAR. TAPES WERE NOT REVIEWED TO SEE WHEN APCH FAILED OR IF JNU TOWER ADVISED ANCHORAGE CENTER OF THE OUTAGE AT COB. DAY 10 TALKING TO MR X HE INFORMED ME THAT THE CTLR DID NOT CONTACT SOC.
