Narrative:
Several interruptions during preflight cockpit set-up. FMS programmed with flight plan departure CEOLA4. Received clearance and did not see revised segment with change to SLOTT2. The captain and I checked route and legs against flight plan and SID page and looked at ACARS printout of clearance but did not detect the revision. Upon climb out; departure control asked what departure we were flying. Again; captain checked our clearance and told controller CEOLA4. Controller handed us off to center without correction or comment and then center queried us. Again captain checked clearance and FMS and told controller we were on CEOLA4. Controller asked if we would accept rerte direct tcc J76 fti then filed route. We accepted and the controller apologized for the confusion. After being handed off to the next sector we realized our mistake of overlooking the revised segment. I should have caught the change and am unsure how I overlooked it at least 2 times. Perhaps a required ACARS acknowledgement; radio call to clearance delivery for entire route verification or new clearance with different squawk code would be helpful.
Original NASA ASRS Text
Title: AN MD-80 FLT CREW DEVIATED FROM THEIR CLEARED SID ROUTING WHEN THEY MISSED THE REVISED SEGMENT PDC.
Narrative: SEVERAL INTERRUPTIONS DURING PREFLT COCKPIT SET-UP. FMS PROGRAMMED WITH FLT PLAN DEP CEOLA4. RECEIVED CLRNC AND DID NOT SEE REVISED SEGMENT WITH CHANGE TO SLOTT2. THE CAPT AND I CHKED RTE AND LEGS AGAINST FLT PLAN AND SID PAGE AND LOOKED AT ACARS PRINTOUT OF CLRNC BUT DID NOT DETECT THE REVISION. UPON CLBOUT; DEP CTL ASKED WHAT DEP WE WERE FLYING. AGAIN; CAPT CHKED OUR CLRNC AND TOLD CTLR CEOLA4. CTLR HANDED US OFF TO CTR WITHOUT CORRECTION OR COMMENT AND THEN CTR QUERIED US. AGAIN CAPT CHKED CLRNC AND FMS AND TOLD CTLR WE WERE ON CEOLA4. CTLR ASKED IF WE WOULD ACCEPT RERTE DIRECT TCC J76 FTI THEN FILED RTE. WE ACCEPTED AND THE CTLR APOLOGIZED FOR THE CONFUSION. AFTER BEING HANDED OFF TO THE NEXT SECTOR WE REALIZED OUR MISTAKE OF OVERLOOKING THE REVISED SEGMENT. I SHOULD HAVE CAUGHT THE CHANGE AND AM UNSURE HOW I OVERLOOKED IT AT LEAST 2 TIMES. PERHAPS A REQUIRED ACARS ACKNOWLEDGEMENT; RADIO CALL TO CLRNC DELIVERY FOR ENTIRE RTE VERIFICATION OR NEW CLRNC WITH DIFFERENT SQUAWK CODE WOULD BE HELPFUL.
