Narrative:

First officer was flying the maxxo departure after takeoff from phx. At 4 DME east of phx VOR we started a right hand turn to 100 degrees when I requested a deviation to left due WX. Departure said; 'turn left to (I believe) 045 degrees to join the st john departure.' I said; 'ok we will change to the st john departure.' phx departure asked if we were flying the maxxo and I said yes. At that time departure said fly the st john and we did. We then went back to the pre departure clearance and it did have the st john right above the maxxo. I missed it; my fault! Departure said no problems and there were no conflicts. Read the departure clearance correctly. It would help out more if they change the SID; don't put the old one on the page. When I looked at the pre departure clearance I saw maxxo and confirmed with release and box; but did not see the st john above. No excuses; I made the mistake.

DEP SAID; 'TURN L TO (I BELIEVE) 045 DEGS TO JOIN THE ST JOHN DEP.' I SAID; 'OK WE WILL CHANGE TO THE ST JOHN DEP.' PHX DEP ASKED IF WE WERE FLYING THE MAXXO AND I SAID YES. AT THAT TIME DEP SAID FLY THE ST JOHN AND WE DID. WE THEN WENT BACK TO THE PDC AND IT DID HAVE THE ST JOHN RIGHT ABOVE THE MAXXO. I MISSED IT; MY FAULT! DEP SAID NO PROBS AND THERE WERE NO CONFLICTS. READ THE DEP CLRNC CORRECTLY. IT WOULD HELP OUT MORE IF THEY CHANGE THE SID; DON'T PUT THE OLD ONE ON THE PAGE. WHEN I LOOKED AT THE PDC I SAW MAXXO AND CONFIRMED WITH RELEASE AND BOX; BUT DID NOT SEE THE ST JOHN ABOVE. NO EXCUSES; I MADE THE MISTAKE.
