Narrative:
Just prior to pushback; crew chief came to cockpit and handed fb a hazmat form. Fb reviewed it and announced the hazmat loaded. I didn't review the form and took the information verbally. During the flight; we discovered the hazmat form was for a different flight on a different day. We contacted dispatch and advised them. They gave us the corrected information.

Title: B777 CAPT REPORTS DEPARTING WITH HAZMAT FOR DIFFERENT ACFT AND DATE. ERROR IS DISCOVERED AND CORRECTED IN FLIGHT.
Narrative: JUST PRIOR TO PUSHBACK; CREW CHIEF CAME TO COCKPIT AND HANDED FB A HAZMAT FORM. FB REVIEWED IT AND ANNOUNCED THE HAZMAT LOADED. I DIDN'T REVIEW THE FORM AND TOOK THE INFO VERBALLY.
