Narrative:

While over haray changing course to direct vxv on the haray two RNAV departure; ZTL asked; 'air carrier X where are going.' as pilot monitoring; I responded; 'direct vxv; landing ZZZ.' the controller replied; the filed route was supposed to continue past haray to zaver on the vxv transition of haray two. The controller issued a heading instruction which was flown until given direct vxv. After establishing the heading; captain; as PF; took over the radios and reported to the controller that our clearance was issued as; the haray two departure direct vxv and that when entered into the FMS the zaver fix was not included as a part of the haray two RNAV departure. The controller complained that zaver was being omitted on a regular basis. The captain assured the controller that the company would be alerted to the situation. The interaction between the captain and the controller was non confrontational; professional; and courteous. Furthermore; there was no traffic observed on TCAS in the area at the time of the alleged route deviation. Originally; the clearance was received from charlotte clearance delivery over the aircraft loudspeaker and both pilots listened to the clearance. The clearance was documented by me as: H2 vxv af 8 230--10. The flight release also reflected haray two vxv dct. The captain entered the route into the FMS using the flight release after having listened to clearance issued by clearance delivery. The clearance was never further questioned about the clearance or route by the crew. The captain never questioned nor discussed the route clearance with the first officer prior to the departure briefing. During this period of preflight preparation the flight attendant distraction the flight deck crew with boarding issues. During the departure briefing both crew members reviewed the route as entered on the flight plan pages and the mileage was checked as acceptable. However; the legs page was not reviewed and compared with the published departure routing as a part of the briefing. The captain believed that the route clearance as issued was haray two direct vxv and it was entered into the FMS by the captain. During the departure briefing the entries on the flight plan page were reviewed and the mileage checked as acceptable; it was not noted by your declarant first officer that the haray two did not include a vxv transition on the flight plan display page. The captain believed the route clearance issued was haray two direct vxv and entered it into the FMS without selecting the presentation option vxv or bna transition. If these entries had been noted by your declarant first officer; they would have prompted further questioning since my understanding of the clearance was to select the vxv transition unless the words haray direct vxv are issued by clearance delivery. The exact wording used by clearance delivery is outside my recollection. However; if entered by your declarant first officer the vxv transition would have been selected based upon my understanding of the route clearance issued. Nonetheless; it was not idented as a potential problem during the route review by your declarant first officer. Consequently; in the future the legs page will be reviewed and compared with the published route to avoid any potential confusion. Callback conversation with reporter revealed the following information: reporter pointed out that the 'documentation' to which he referred was an air carrier provided form on which flight crews enter pertinent flight documentation by hand; ATIS; clrncs; weight and balance data; etc. He felt he wrote down the verbal clearance exactly as he heard it but emphasized he could not recall the exact verbiage; in particular whether 'vxv' was followed by the word 'transition. He also stressed the fact that this was the flight crew's first flight together and the distractions he mentions had made it difficult to get a 'feel' for each others cockpit manner and/or relative facility with the processes involved. He was confident that if he had programmed the clearance as received; rather than the captain; he would have selected the 'trans' page of the FMS to ascertain whether vxv was a selectable transition to the haray departure. He did not; however; question the captain's programming or departure briefing.

Title: ACR FLIES DIRECT TO VXV VICE VIA VXV TRANSITION ON THE HARAY RNAV SID FROM CLT.

Narrative: WHILE OVER HARAY CHANGING COURSE TO DIRECT VXV ON THE HARAY TWO RNAV DEP; ZTL ASKED; 'ACR X WHERE ARE GOING.' AS PLT MONITORING; I RESPONDED; 'DIRECT VXV; LNDG ZZZ.' THE CTLR REPLIED; THE FILED RTE WAS SUPPOSED TO CONTINUE PAST HARAY TO ZAVER ON THE VXV TRANSITION OF HARAY TWO. THE CTLR ISSUED A HDG INSTRUCTION WHICH WAS FLOWN UNTIL GIVEN DIRECT VXV. AFTER ESTABLISHING THE HDG; CAPT; AS PF; TOOK OVER THE RADIOS AND RPTED TO THE CTLR THAT OUR CLRNC WAS ISSUED AS; THE HARAY TWO DEP DIRECT VXV AND THAT WHEN ENTERED INTO THE FMS THE ZAVER FIX WAS NOT INCLUDED AS A PART OF THE HARAY TWO RNAV DEP. THE CTLR COMPLAINED THAT ZAVER WAS BEING OMITTED ON A REGULAR BASIS. THE CAPT ASSURED THE CTLR THAT THE COMPANY WOULD BE ALERTED TO THE SIT. THE INTERACTION BTWN THE CAPT AND THE CTLR WAS NON CONFRONTATIONAL; PROFESSIONAL; AND COURTEOUS. FURTHERMORE; THERE WAS NO TFC OBSERVED ON TCAS IN THE AREA AT THE TIME OF THE ALLEGED RTE DEV. ORIGINALLY; THE CLRNC WAS RECEIVED FROM CHARLOTTE CLRNC DELIVERY OVER THE ACFT LOUDSPEAKER AND BOTH PLTS LISTENED TO THE CLRNC. THE CLRNC WAS DOCUMENTED BY ME AS: H2 VXV AF 8 230--10. THE FLT RELEASE ALSO REFLECTED HARAY TWO VXV DCT. THE CAPT ENTERED THE RTE INTO THE FMS USING THE FLT RELEASE AFTER HAVING LISTENED TO CLRNC ISSUED BY CLRNC DELIVERY. THE CLRNC WAS NEVER FURTHER QUESTIONED ABOUT THE CLRNC OR RTE BY THE CREW. THE CAPT NEVER QUESTIONED NOR DISCUSSED THE RTE CLRNC WITH THE FO PRIOR TO THE DEP BRIEFING. DURING THE DEP BRIEFING THE ENTRIES ON THE FLT PLAN PAGE WERE REVIEWED AND THE MILEAGE CHKED AS ACCEPTABLE; IT WAS NOT NOTED BY YOUR DECLARANT FO THAT THE HARAY TWO DID NOT INCLUDE A VXV TRANSITION ON THE FLT PLAN DISPLAY PAGE. THE CAPT BELIEVED THE RTE CLRNC ISSUED WAS HARAY TWO DIRECT VXV AND ENTERED IT INTO THE FMS WITHOUT SELECTING THE PRESENTATION OPTION VXV OR BNA TRANSITION. IF THESE ENTRIES HAD BEEN NOTED BY YOUR DECLARANT FO; THEY WOULD HAVE PROMPTED FURTHER QUESTIONING SINCE MY UNDERSTANDING OF THE CLRNC WAS TO SELECT THE VXV TRANSITION UNLESS THE WORDS HARAY DIRECT VXV ARE ISSUED BY CLRNC DELIVERY. THE EXACT WORDING USED BY CLRNC DELIVERY IS OUTSIDE MY RECOLLECTION. HOWEVER; IF ENTERED BY YOUR DECLARANT FO THE VXV TRANSITION WOULD HAVE BEEN SELECTED BASED UPON MY UNDERSTANDING OF THE RTE CLRNC ISSUED. NONETHELESS; IT WAS NOT IDENTED AS A POTENTIAL PROB DURING THE RTE REVIEW BY YOUR DECLARANT FO. CONSEQUENTLY; IN THE FUTURE THE LEGS PAGE WILL BE REVIEWED AND COMPARED WITH THE PUBLISHED RTE TO AVOID ANY POTENTIAL CONFUSION. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: REPORTER POINTED OUT THAT THE 'DOCUMENTATION' TO WHICH HE REFERRED WAS AN ACR PROVIDED FORM ON WHICH FLT CREWS ENTER PERTINENT FLT DOCUMENTATION BY HAND; ATIS; CLRNCS; WT AND BALANCE DATA; ETC. HE FELT HE WROTE DOWN THE VERBAL CLRNC EXACTLY AS HE HEARD IT BUT EMPHASIZED HE COULD NOT RECALL THE EXACT VERBIAGE; IN PARTICULAR WHETHER 'VXV' WAS FOLLOWED BY THE WORD 'TRANSITION. HE ALSO STRESSED THE FACT THAT THIS WAS THE FLT CREW'S FIRST FLT TOGETHER AND THE DISTRACTIONS HE MENTIONS HAD MADE IT DIFFICULT TO GET A 'FEEL' FOR EACH OTHERS COCKPIT MANNER AND/OR RELATIVE FACILITY WITH THE PROCESSES INVOLVED. HE WAS CONFIDENT THAT IF HE HAD PROGRAMMED THE CLRNC AS RECEIVED; RATHER THAN THE CAPTAIN; HE WOULD HAVE SELECTED THE 'TRANS' PAGE OF THE FMS TO ASCERTAIN WHETHER VXV WAS A SELECTABLE TRANSITION TO THE HARAY DEP. HE DID NOT; HOWEVER; QUESTION THE CAPTAIN'S PROGRAMMING OR DEP BRIEFING.
