Narrative:

Cessna 414 called; requested IFR departure clearance from I6's (mt sterling; il) to ZZZ. I issued the clearance as filed and assigned 15000 ft and the correct squawk at XA00Z. At about XB00Z; peoria approach called and said pointout approved on specific code climbing. I responded that 'I don't know who that was but I thought ZAU was working the aircraft VFR.' at XA00Z brl lo sector in ZAU called and asked about aircraft X. They had a code near pia at 15000 ft and thought I forgot to make a handoff. I said I didn't clear him and I thought they had done it. I asked for a radio check from aircraft X and he said loud and clear. I asked if he was given a clearance and he said affirmative but he was not radar idented. I called aircraft X radar contact and made the departure entry into the computer. I then handed off aircraft X and changed his frequency to ZAU. I forgot that I gave the clearance and didn't enter the departure message in the NAS computer. Traffic was light and there were no mitigating circumstances.

Narrative: CESSNA 414 CALLED; REQUESTED IFR DEP CLRNC FROM I6'S (MT STERLING; IL) TO ZZZ. I ISSUED THE CLRNC AS FILED AND ASSIGNED 15000 FT AND THE CORRECT SQUAWK AT XA00Z. AT ABOUT XB00Z; PEORIA APCH CALLED AND SAID POINTOUT APPROVED ON SPECIFIC CODE CLBING. I RESPONDED THAT 'I DON'T KNOW WHO THAT WAS BUT I THOUGHT ZAU WAS WORKING THE ACFT VFR.' AT XA00Z BRL LO SECTOR IN ZAU CALLED AND ASKED ABOUT ACFT X. THEY HAD A CODE NEAR PIA AT 15000 FT AND THOUGHT I FORGOT TO MAKE A HDOF. I SAID I DIDN'T CLR HIM AND I THOUGHT THEY HAD DONE IT. I ASKED FOR A RADIO CHK FROM ACFT X AND HE SAID LOUD AND CLR. I ASKED IF HE WAS GIVEN A CLRNC AND HE SAID AFFIRMATIVE BUT HE WAS NOT RADAR IDENTED. I CALLED ACFT X RADAR CONTACT AND MADE THE DEP ENTRY INTO THE COMPUTER. I THEN HANDED OFF ACFT X AND CHANGED HIS FREQ TO ZAU. I FORGOT THAT I GAVE THE CLRNC AND DIDN'T ENTER THE DEP MESSAGE IN THE NAS COMPUTER. TFC WAS LIGHT AND THERE WERE NO MITIGATING CIRCUMSTANCES.
