Narrative:
We had just taxied from runway 09L. Tower gave us a ground frequency that was not on the commercial chart page; which resulted in some confusion. Then we were given very confusing taxi-in instructions that were not at all normal or intuitive. This required turns away from the terminal and taxi on to 2 runways crossing another. All this after a very early get up on a short layover in a noisy hotel room. I was fatigued. We were told to taxi from Z to T on T2 onto runway 14R. The ground controller even asked if they could make it any more confusing? Right about the time I passed T2 I came almost nose to nose with outbound traffic on T at H. Ground control instructed me to turn right on T3 then 14R no more problems. But we had taxied from 09L to 22R down it to Z crossing 14L. This confusing operation coupled with the poor numbering system of 09L 09R on the north side of the field with 04L and 04R being on the south side of the field - you are going to have a bad crash!!! Ground and tower frequencies not listed on the commercial chart page. This is bad. Couple that with fatigue you're in for a bad accident.
