Narrative:
We were delayed due to maintenance investigating a hydraulic leak. When we were released; we taxied out and took off. Upon raising the gear; we received a 'gear disagree' EICAS message and the accompanying 'gear' amber light and the master caution. We accomplished the irregular procedure; contacted dispatch and maintenance; issued a cabin advisory; declared an emergency; and returned to departure airport and landed over structural landing weight by 7200LBS. The landing was smooth.

Title: A B757's landing gear did not retract after takeoff when the landing gear handle was raised. The 'Gear Disagree' EICAS alerted; an emergency was declared with a return to land.
