Narrative:

The copy of the computer generated and filed aftn flight plan for large transport flight 3/thu was picked up with the pre-departure papers. It showed the following route had been filed to regional southwest airport, fl: jfk-cyn-J37-gve-J75-tay-J85-gnv. When contacted jfk clearance delivery cleared us as follows: 'large transport is cleared to the rsw airport via the jfk #1 departure, breezy point climb, rbv transition, as filed, expect FL310, squawk XXXX.' after departure the SID was complied with while in contact with jfk departure, and also with ny departure. While on the SID designated right of the canarsie VOR, large transport was cleared direct to rbv VOR and to contact ny center. Ny center cleared the flight to FL310. After passing rbv VOR the copilot, who was flying, began a turn toward cyn VOR (approximately a 180 degree heading, and 75 degree heading change). At about XA05Z, passing through about FL205 and 210 degrees on the heading, the center called and asked for our heading. When I replied, the center issued an 'immediate right turn to heading 330, maintain FL220! Where are you going?!' I replied that we were going to cyn VOR as filed. We were then issued a heading of 360 degrees to intercept the rbv VOR 274 right, and stand by to copy a reroute. The new clearance was: 'cleared to rsw airport via J230, J75 tay, J85 gnv, direct lal, direct fmy, direct rsw.' after reading back the new clearance, I read the controller the original clearance and explained once again that we had been cleared as filed. He explained about the expanded east coast plan and that it had been in effect for a week, and that the company computer had not as yet begun to file the new flight plans. As a result we should have been given a full readback clearance by jfk clearance delivery. He questioned the fact that we had not been given a full readback. I assured him we had not. A short time later the controller called and told us that the error had occurred between the center and the tower, and they had confirmed that we indeed had not been given the revised routing. It has been my observation that it usually takes the combination of at least 3 events to bring about a serious problem. On this flight, 2 of the boxes had been filled, and had the center controller been too busy or not observant at the right time, there was certainly the potential for a very serious problem.

IT SHOWED THE FOLLOWING ROUTE HAD BEEN FILED TO REGIONAL SW ARPT, FL: JFK-CYN-J37-GVE-J75-TAY-J85-GNV. WHEN CONTACTED JFK CLRNC DELIVERY CLRED US AS FOLLOWS: 'LGT IS CLRED TO THE RSW ARPT VIA THE JFK #1 DEP, BREEZY POINT CLB, RBV TRANSITION, AS FILED, EXPECT FL310, SQUAWK XXXX.' AFTER DEP THE SID WAS COMPLIED WITH WHILE IN CONTACT WITH JFK DEP, AND ALSO WITH NY DEP. WHILE ON THE SID DESIGNATED R OF THE CANARSIE VOR, LGT WAS CLRED DIRECT TO RBV VOR AND TO CONTACT NY CENTER. NY CENTER CLRED THE FLT TO FL310. AFTER PASSING RBV VOR THE COPLT, WHO WAS FLYING, BEGAN A TURN TOWARD CYN VOR (APPROX A 180 DEG HDG, AND 75 DEG HDG CHANGE). AT ABOUT XA05Z, PASSING THROUGH ABOUT FL205 AND 210 DEGS ON THE HDG, THE CENTER CALLED AND ASKED FOR OUR HDG. WHEN I REPLIED, THE CENTER ISSUED AN 'IMMEDIATE RIGHT TURN TO HDG 330, MAINTAIN FL220! WHERE ARE YOU GOING?!' I REPLIED THAT WE WERE GOING TO CYN VOR AS FILED. WE WERE THEN ISSUED A HDG OF 360 DEGS TO INTERCEPT THE RBV VOR 274 R, AND STAND BY TO COPY A REROUTE. THE NEW CLRNC WAS: 'CLRED TO RSW ARPT VIA J230, J75 TAY, J85 GNV, DIRECT LAL, DIRECT FMY, DIRECT RSW.' AFTER READING BACK THE NEW CLRNC, I READ THE CTLR THE ORIGINAL CLRNC AND EXPLAINED ONCE AGAIN THAT WE HAD BEEN CLRED AS FILED. HE EXPLAINED ABOUT THE EXPANDED EAST COAST PLAN AND THAT IT HAD BEEN IN EFFECT FOR A WEEK, AND THAT THE COMPANY COMPUTER HAD NOT AS YET BEGUN TO FILE THE NEW FLT PLANS. AS A RESULT WE SHOULD HAVE BEEN GIVEN A FULL READBACK CLRNC BY JFK CLRNC DELIVERY. HE QUESTIONED THE FACT THAT WE HAD NOT BEEN GIVEN A FULL READBACK. I ASSURED HIM WE HAD NOT. A SHORT TIME LATER THE CTLR CALLED AND TOLD US THAT THE ERROR HAD OCCURRED BTWN THE CENTER AND THE TWR, AND THEY HAD CONFIRMED THAT WE INDEED HAD NOT BEEN GIVEN THE REVISED ROUTING. IT HAS BEEN MY OBSERVATION THAT IT USUALLY TAKES THE COMBINATION OF AT LEAST 3 EVENTS TO BRING ABOUT A SERIOUS PROB. ON THIS FLT, 2 OF THE BOXES HAD BEEN FILLED, AND HAD THE CENTER CTLR BEEN TOO BUSY OR NOT OBSERVANT AT THE RIGHT TIME, THERE WAS CERTAINLY THE POTENTIAL FOR A VERY SERIOUS PROB.
