Narrative:
On the runway in takeoff position we were cleared for takeoff; direct gabar; no other stipulation. There is no direction stated in airport detailed pages or anywhere else. The first officer repeated back cleared for takeoff runway heading. Tower came back 'direct gabar' then after a short delay said 'right turnout.' after takeoff; visibility was restricted and we determined the short way; and terrain cognizant way was to go left. There was some confusion on what tower had intended and tower was difficult to understand and very slow to respond. So; we turned left direct gabar. We didn't receive any objection or comments otherwise from ATC. Fight continued uneventfully.
