Narrative:
Captain was flying and called for gear extension 1NM prior to FAF. Main gear extended normally with no nose gear and the 'gear disagree' and 'gear door' EICAS messages. Captain instructed me to cycle the gear. I hesitated and told him I thought we should get the checklist. He told me to cycle the gear again so I cycled the gear with no change in indication. We executed a missed approach and asked tower to look at our nose gear as we passed by the tower. They stated doors were open and nose gear still in the well. Climbed to 4000 MSL and got delay vectors to northwest of our departure airport. Captain still flying and I got into checklist. Followed checklist; which had us do the alternate gear extension. Nose gear extended and got good indication. Returned to our departure airport uneventfully. Landed with 4600 pounds.

Title: B757-200 nose gear failed to extend. Flight crew recycled gear to no avail; made a go-around; completed the alternate gear extension procedure successfully and landed uneventfully.
