Narrative:
Incorrect fueling (not enough) by fueler. First officer and captain distracted by FAA inspectors on board aircraft. The captain failed to recheck fuel slip against the fuel gauges at departure time. Aircraft departed bos for den with insufficient fuel for trip. On climbout, error was discovered after check WX with bos center, a fuel stop was planned and completed at buf. Supplemental information from acn 86232: fueler refueled aircraft for flight but for some reason stopped short of the release fuel load. He gave the crew the fuel slip which showed the correct amount as having been added. The captain missed this. The fueler came up and handed me the fuel slip. I checked this for proper equipment and flight number. As I had not seen the release, I was not aware of our dispatch for release load, however the amount of 34,500 seemed appropriate. During climb we exhausted center tank fuel at which time we discovered that while we had been fueled it was not to the release load of 34,500 pounds.
Original NASA ASRS Text
Title: ACR MLG DEP BOS WITHOUT SUFFICIENT FUEL TO COMPLETE FLT TO DEN. AT TOP OF CLIMB THE FLT CREW DISCOVERED THE ERROR AND DIVERTED TO ALTERNATE FOR ADDITIONAL FUEL.
Narrative: INCORRECT FUELING (NOT ENOUGH) BY FUELER. F/O AND CAPT DISTRACTED BY FAA INSPECTORS ON BOARD ACFT. THE CAPT FAILED TO RECHECK FUEL SLIP AGAINST THE FUEL GAUGES AT DEP TIME. ACFT DEPARTED BOS FOR DEN WITH INSUFFICIENT FUEL FOR TRIP. ON CLIMBOUT, ERROR WAS DISCOVERED AFTER CHECK WX WITH BOS CENTER, A FUEL STOP WAS PLANNED AND COMPLETED AT BUF. SUPPLEMENTAL INFORMATION FROM ACN 86232: FUELER REFUELED ACFT FOR FLT BUT FOR SOME REASON STOPPED SHORT OF THE RELEASE FUEL LOAD. HE GAVE THE CREW THE FUEL SLIP WHICH SHOWED THE CORRECT AMOUNT AS HAVING BEEN ADDED. THE CAPT MISSED THIS. THE FUELER CAME UP AND HANDED ME THE FUEL SLIP. I CHECKED THIS FOR PROPER EQUIP AND FLT NUMBER. AS I HAD NOT SEEN THE RELEASE, I WAS NOT AWARE OF OUR DISPATCH FOR RELEASE LOAD, HOWEVER THE AMOUNT OF 34,500 SEEMED APPROPRIATE. DURING CLIMB WE EXHAUSTED CENTER TANK FUEL AT WHICH TIME WE DISCOVERED THAT WHILE WE HAD BEEN FUELED IT WAS NOT TO THE RELEASE LOAD OF 34,500 LBS.
