Narrative:

We closed the main cabin door; advised the flight deck 'cabin secured' and closed the flight deck door; and I heard the click check to make sure it was locked. We pushed back and taxied out to the runway; started our take off roll; that's when the flight deck door flew open; my right hand went right for my seatbelt buckle ready to defend the flight deck. Once we were in the air I waited 60 seconds then got up and closed the door; I had my back to the door facing the cabin I heard the click then tugged on the door and it was locked. I stepped away from the door; that's when the door fell in towards the forward galley and hit me in the back of the head; within 2 seconds the other flight attendant was in the forward galley to assist me by blocking the forward galley. The captain looked towards the door and realized that the door was off the hinges; he then contacted us via the phone and requested us to have the jumpseater move to row 1. The captain also requested that the other flight attendant remain in the forward part of the cabin for landing. I then made a PA instructing passengers to remain seated and that we were returning due to the flight deck door coming off the hinges. I moved the door to 1D and secured it with the seat belt and an extender. I remained at the flight deck door blocking it with the jumpseater seated in 1X. I instructed the other flight attendant to prepare the cabin for landing; and we remained standing till about a thousand feet; the other flight attendant took seat 5B to block the aisle and I took jump 1L. Once on the ground and we turned off the runway the other flight attendant returned to the aft of the aircraft.

Title: An ERJ-170 Flight Attendant reported being struck in the head when the cockpit door came off its hinges in flight and fell into the cabin.
