Narrative:

I was vectoring an air carrier for an instrument approach when the primary and secondary targets disappeared. The aircraft was 15 miles west of the airport when it disappeared and came back less than 10 miles from the field. This happened on a critical phase of the flight when the aircraft was descending and being vectored to final. The facility knows about these types of incidents happening and they have created a 'WX and radar discrepancy report' form for us to identify when this happens. Recommendation: need to fix the radar problem immediately.

Title: ZSU Controller expressed concern regarding the disappearance of both the Primary and Secondary RADAR targets while vectoring an aircraft to the final approach; noting the RADAR problem is on-going.
