Narrative:

Aircraft X called airborne VFR requesting IFR clearance. Aircraft X was issued IFR clearance to maintain 70. Aircraft Y was level 80 passing overhead. Aircraft X mode C readout indicating climbing through 70, altimeter issued, verification of altitude proved different by 1000' aircraft told to stop altitude squawk. Subsequently aircraft X observed traffic overhead. Verification of altimeter indicated pilot had wrong altimeter setting dialed in thus causing aircraft X and aircraft Y to be at same altitude. Aircraft X was told to maintain visual sep, maintain 70.

Title: LESS THAN STANDARD SEPARATION BETWEEN SMA AND SMT. BOTH ACFT IFR FLT PLAN. OPERATIONAL ERROR.

Narrative: ACFT X CALLED AIRBORNE VFR REQUESTING IFR CLRNC. ACFT X WAS ISSUED IFR CLRNC TO MAINTAIN 70. ACFT Y WAS LEVEL 80 PASSING OVERHEAD. ACFT X MODE C READOUT INDICATING CLBING THROUGH 70, ALTIMETER ISSUED, VERIFICATION OF ALT PROVED DIFFERENT BY 1000' ACFT TOLD TO STOP ALT SQUAWK. SUBSEQUENTLY ACFT X OBSERVED TFC OVERHEAD. VERIFICATION OF ALTIMETER INDICATED PLT HAD WRONG ALTIMETER SETTING DIALED IN THUS CAUSING ACFT X AND ACFT Y TO BE AT SAME ALT. ACFT X WAS TOLD TO MAINTAIN VISUAL SEP, MAINTAIN 70.
