Narrative:
We just leveled off at 4;000 and intercepted localizer for visual approach. Flaps were at 1. As I slowed through 200 KTS; called for flaps 5; first officer placed the flap handle at 5; but the flap indicator showed flaps 15 with the handle still at 5. EICAS 'te flaps disagree' came on; along with the status message 'flap hyd shutoff' or something similar to it. We exited the approach and ran the appropriate checklist; which had us extend the flaps using the alternate method and land with flaps 20. Checked the landing distance charts for irregular configuration. After a conference call with dispatch and maintenance; we decided to declare an emergency for partial flap landing. [We performed a] normal landing on the longest runway and taxied to the gate.
