Narrative:
Cruising at FL370. There was a sudden bang followed by heavy vibration. Number 3 engine noted to have failed. Requested lower altitude from center and began gradual descent. Completed checklist to secure failed engine. Vibration decreased markedly with engine shutdown. Stabilized at FL290. Contacted company maintenance and checked WX at dulles and lga. As WX at lga was good and improving and after talking to maintenance the decision was made to continue to lga. We were headed for dulles at that point and so had to change our flight plan for a second time back to lga. Atlanta and washington center very helpful, very professional. Flight terminated with very uneventful landing in lga. Engine failure apparently due to leaking forward lavatory forming ice ball on outside of aircraft which broke off and was ingested by the #3 engine.

Title: ACR LGT FOREIGN OBJECT INGESTION ICE. INFLT ENGINE SHUTDOWN THEN CONTINUED TO DESTINATION ARPT.

AS WX AT LGA WAS GOOD AND IMPROVING AND AFTER TALKING TO MAINT THE DECISION WAS MADE TO CONTINUE TO LGA. WE WERE HEADED FOR DULLES AT THAT POINT AND SO HAD TO CHANGE OUR FLT PLAN FOR A SECOND TIME BACK TO LGA. ATLANTA AND WASHINGTON CTR VERY HELPFUL, VERY PROFESSIONAL. FLT TERMINATED WITH VERY UNEVENTFUL LNDG IN LGA. ENG FAILURE APPARENTLY DUE TO LEAKING FORWARD LAVATORY FORMING ICE BALL ON OUTSIDE OF ACFT WHICH BROKE OFF AND WAS INGESTED BY THE #3 ENG.
