Narrative:

We took off out of jax for tpa. We were handed off to departure who said, 'radar contact fly heading 210, climb and maintain 11000.' upon reaching 11000' the first officer asked for higher. The controller gave us a center frequency which was incorrect. Our position was approximately 15 mi northeast of gnv. Center found the appropriate frequency and issued it to us. This controller told us to squawk our departure frequency, at which point the first officer discovered he had neglected to turn it on. We were then told to 'radar contact,' and given climb clearance. We told this controller that we had neglected to turn on the transponder and that apparently jax departure just forgot about us after our initial call. We may have clipped a restr area on the 210 degree heading out of jax.

Narrative: WE TOOK OFF OUT OF JAX FOR TPA. WE WERE HANDED OFF TO DEP WHO SAID, 'RADAR CONTACT FLY HDG 210, CLB AND MAINTAIN 11000.' UPON REACHING 11000' THE F/O ASKED FOR HIGHER. THE CTLR GAVE US A CENTER FREQ WHICH WAS INCORRECT. OUR POS WAS APPROX 15 MI NE OF GNV. CENTER FOUND THE APPROPRIATE FREQ AND ISSUED IT TO US. THIS CTLR TOLD US TO SQUAWK OUR DEP FREQ, AT WHICH POINT THE F/O DISCOVERED HE HAD NEGLECTED TO TURN IT ON. WE WERE THEN TOLD TO 'RADAR CONTACT,' AND GIVEN CLB CLRNC. WE TOLD THIS CTLR THAT WE HAD NEGLECTED TO TURN ON THE TRANSPONDER AND THAT APPARENTLY JAX DEP JUST FORGOT ABOUT US AFTER OUR INITIAL CALL. WE MAY HAVE CLIPPED A RESTR AREA ON THE 210 DEG HDG OUT OF JAX.
