Narrative:

Weather forecast [for our midwest destination] was 1;000 ft broken 2 NM light snow for time of arrival. At 10;000 ft approach advised light to moderate rime [ice] on approach. Encountered rime at 7;000 ft. At 4;000 ft moderate rime [encountered] 10 NM from fap. 5 NM from fap encountered heavy to severe mixed icing. Cockpit side windows became opaque due [to] ice. [There was] tennis ball sized ice buildup on wipers and windshield center post; [and] ice covered 1/3 [of] forward windshield. Advised ATC; [then] switched to tower. Tower reported good to fair braking [action for] first half [of runway]; fair to poor [for the] last half. Maximum auto-brakes [were] selected. At 200 ft; [the aircraft] pitched forward severely; go-around throttle applied; 50 ft call occurred at [the] middle of the approach lights; maximum throttle applied; [and] we just barely made the runway. Wing and engine anti-ice were on; engines above 50% at all times. Even with maximum auto-brakes we almost ran off the runway at far end. Braking was nil the last 1;500 ft and turnoff. [We] informed tower and dispatch of [the] conditions after turnoff. Aircraft behind us declared icing emergency. On arrival at gate; [we] found the entire aircraft [to be] covered with clear ice 1 to 2 inches thick; [it] looked like a popsicle. Engines had large ice spikes extending several inches on center cowls and fan blades. Maintenance [was] informed; [and they] checked for damage.

Title: A B757-200 encountered severe ice on final approach that overwhelmed the anti-ice systems. An attempted go-around at 200 FT AGL was unsuccessful because of decreased available performance and the aircraft just reached the runway with max thrust applied.
