Narrative:
Passenger door-2L door slide was removed during a heavy check visit. During that removal; it was discovered that a 'bolt with attached yellow streamer' (safety pin from original shipment) was installed in the slide pack center release mechanism.

Title: A Mechanic was informed that a B747 passenger door slide he had installed at cabin Door 2-L approximately eighteen months earlier; was found with an unapproved bolt and yellow paper streamer installed in the pack center release mechanism. The bolt had deactivated the slide and prevented emergency egress from that door.
