Narrative:
In 1985, I believe, I was convicted of driving under the influence of alcohol. I paid the fine, went to dui classes and that was the end of it. The medical form asks if you have been convicted of such an offense and I don't recall putting down the 1985 conviction. It's not as if I'm trying to lie or cover it up. I just can't recall putting down the event. You can bet I'll put it down on the next physical in 12/88. The consequences of the occurrence have no direct effect on my job as I am self employed and don't have a boss to report to as such. The people I contract to fly don't exercise the right to hire or fire me even in the event they knew of the conviction. I doubt it would concern them as my personal conduct has never been brought to the forefront. I have just seen on the national tv news where the FAA is indicting pilots for having lied on the form, but it appears for indictments involving other than alcohol. Callback conversation with reporter revealed the following: reporter shocked to see pilots being taken away in handcuffs. Realized possible results of his neglect to complete form correctly. Contract pilot flying for corp clients so no real concern re: employers.

Title: INCOMPLETE INFORMATION ON MEDICAL FORM.
Narrative: IN 1985, I BELIEVE, I WAS CONVICTED OF DRIVING UNDER THE INFLUENCE OF ALCOHOL. I PAID THE FINE, WENT TO DUI CLASSES AND THAT WAS THE END OF IT. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING: RPTR SHOCKED TO SEE PLTS BEING TAKEN AWAY IN HANDCUFFS. REALIZED POSSIBLE RESULTS OF HIS NEGLECT TO COMPLETE FORM CORRECTLY. CONTRACT PLT FLYING FOR CORP CLIENTS SO NO REAL CONCERN RE: EMPLOYERS.
