Narrative:

[We were] flying the grahm arrival into bna. Landing north arrival states expect radar vectors after grahm VOR. We did not receive an assigned heading after passing grahm. Very small note specifies routing of the bna 246 radial while ball note and landing box say expect vectors. We had received two runway changes close to grahm. In executing these; the purple line shifted away from the ghm 066; as depicted on the arrival; to a fix on the runway 2C ILS. We tracked toward this fix instead. As pilot flying; I failed to pick this up. ATC queried us and then gave us a vector to final.

Title: An MD83 flight crew failed to reconcile a Ball Note on their commercially provided GRAHAM FIVE ARRIVAL to BNA advising to 'EXPECT' vectors...after passing GHM' with a 'ROUTING' line immediately above advising they were to proceed 'From over GHM via the BNA R-246.' A brief track deviation occurred prior to recognition and rerouting by Approach Control.
