Narrative:
During cruise; the lead flight attendant informed me that the 'flight attendants prepare for takeoff' PA was not heard and that some flight attendants were not seated at the start of the takeoff roll as a result. She later informed me that one flight attendant injured her ankle. On arrival; talking to approach; I made the 'prepare for landing' PA and subsequently was not able to be heard by ATC. I then drew the conclusion that the PA was not heard due to microphone selector malfunction. All other sops and fars were followed. In the future; I will be sure I hear my side tone on all transmissions to make sure they went out properly..

Title: A B757 First Officer's microphone selector malfunctioned; unknown to the crew. A Flight Attendant was injured during takeoff because takeoff alert was not heard. Near the destination ATC also did not receive an initial call.
