Narrative:

The incident had its beginning 6 months prior when I destroyed my medical in the washing machine by accident. The fastest way at the time for me to have a medical was to get a new one. This threw me out of a well established pattern of getting medicals in feb and aug. I had one more chance to catch it before it was too late as my company published a list of due dates for check rides and medicals, I didn't check. I was made aware of this mistake by the company training and records section of flight operations when they asked me for the photo copy of my new medical, (my first thought was, what new medical?!). Lesson learned, it's a good idea to open up those pieces of paper on a regular basis and see if they say what you think they say, ie check the document's content as well as their physical presence.

Title: EXPIRED MEDICAL.

I WAS MADE AWARE OF THIS MISTAKE BY THE COMPANY TRAINING AND RECORDS SECTION OF FLT OPS WHEN THEY ASKED ME FOR THE PHOTO COPY OF MY NEW MEDICAL, (MY FIRST THOUGHT WAS, WHAT NEW MEDICAL?!). LESSON LEARNED, IT'S A GOOD IDEA TO OPEN UP THOSE PIECES OF PAPER ON A REGULAR BASIS AND SEE IF THEY SAY WHAT YOU THINK THEY SAY, IE CHK THE DOCUMENT'S CONTENT AS WELL AS THEIR PHYSICAL PRESENCE.
