Narrative:
MTR X was on ILS approach. Small aircraft Y was released for IFR departure. Small aircraft Y cleared for takeoff with initial heading of 200 degrees. X had been issued missed approach instruction of 260 degrees. Approach control advised tower to issue 190 degree heading. Tower issued, 'fly heading 190 degrees and contact approach.' X did not verbally acknowledge tower visually watched MTR X. Aircraft appeared to be on correct heading until out of visibility range. X turned to 260 degrees south of the airport and switched frequencys. Approximately 3 mi southwest of airport X target and Y target merged. X was issued new heading and climb instruction and advised of traffic in immediate vicinity. To avoid recurrence: never issue heading or altitude instructions with frequency change instructions in same transmission. Always get verbal acknowledgement of such instructions. If none are received, coordinate with receiving facility. Callback conversation with reporter revealed the following: reporter gave ATC experience. Not sure how close aircraft got to each other. Altitude of both was approximately 3000'. Aircraft Y had departed runway 17. Military aircraft was on UHF. Local controller was transmitting UHF VHF simultaneously. Pwa tower has BRITE scope from okc radar.

Narrative: MTR X WAS ON ILS APCH. SMA Y WAS RELEASED FOR IFR DEP. SMA Y CLRED FOR TKOF WITH INITIAL HDG OF 200 DEGS. X HAD BEEN ISSUED MISSED APCH INSTRUCTION OF 260 DEGS. APCH CTL ADVISED TWR TO ISSUE 190 DEG HDG. TWR ISSUED, 'FLY HDG 190 DEGS AND CONTACT APCH.' X DID NOT VERBALLY ACKNOWLEDGE TWR VISUALLY WATCHED MTR X. ACFT APPEARED TO BE ON CORRECT HDG UNTIL OUT OF VIS RANGE. X TURNED TO 260 DEGS S OF THE ARPT AND SWITCHED FREQS. APPROX 3 MI SW OF ARPT X TARGET AND Y TARGET MERGED. X WAS ISSUED NEW HDG AND CLB INSTRUCTION AND ADVISED OF TFC IN IMMEDIATE VICINITY. TO AVOID RECURRENCE: NEVER ISSUE HDG OR ALT INSTRUCTIONS WITH FREQ CHANGE INSTRUCTIONS IN SAME XMISSION. ALWAYS GET VERBAL ACKNOWLEDGEMENT OF SUCH INSTRUCTIONS. IF NONE ARE RECEIVED, COORDINATE WITH RECEIVING FAC. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING: RPTR GAVE ATC EXPERIENCE. PWA TWR HAS BRITE SCOPE FROM OKC RADAR.
