Narrative:
Small aircraft X ignored ATC instructions to hold short of txwys and taxied out to the active runway on his own, after acknowledging the hold instructions. This information was immediately given to the controller in charge. This information was not logged and management would not pursue this incident even though they agreed an far was violated. Local control and ground control were combined at this time and the situation this caused on ground control prevented full attention to be given to the aircraft being worked at local control.

Title: SMA TAXIED TO AN ACTIVE RWY AFTER BEING INSTRUCTED TO HOLD SHORT OF TXWY. NO OTHER TRAFFIC INVOLVED.
Narrative: SMA X IGNORED ATC INSTRUCTIONS TO HOLD SHORT OF TXWYS AND TAXIED OUT TO THE ACTIVE RWY ON HIS OWN, AFTER ACKNOWLEDGING THE HOLD INSTRUCTIONS. THIS INFO WAS IMMEDIATELY GIVEN TO THE CIC. THIS INFO WAS NOT LOGGED AND MGMNT WOULD NOT PURSUE THIS INCIDENT EVEN THOUGH THEY AGREED AN FAR WAS VIOLATED. LCL CTL AND GND CTL WERE COMBINED AT THIS TIME AND THE SITUATION THIS CAUSED ON GND CTL PREVENTED FULL ATTN TO BE GIVEN TO THE ACFT BEING WORKED AT LCL CTL.
