Narrative:
I was working air carrier X which departed laf IFR to ord climbing to 8000'. I gave him radar contact out of 5000' for 8000', but did not start a data block. The aircraft was on the wrong code also which caused the computer to not automatic-acquire a data block. The aircraft entered chicago center airspace without a handoff.
