Narrative:
The fueler began fueling the aircraft immediately upon arrival; even though the passenger loading bridge was not placed up to the aircraft for approximately five minutes. Had an incident occurred; there would have been no egress possible from the plane; resulting in loss of life as well as damage to; or loss of; the aircraft. The fueler was also observed blocking the deadman [control] open and not adequately monitoring the gauges during the fueling process.
