Narrative:
Passed lyh VOR, had lynchburg airport visually, cleared visual approach by center. First officer reported position as '5 south of the VOR' (meant 5 south of the airport). I didn't catch it. Requested/cleared to enter left downwind runway 21. Caught yellow aircraft in center of windscreen, same altitude. Executed hard right turn, he did the same. Missed by 100-200'. Tower said they didn't expect us to be where we were. (People on ground saw near miss but tower was unaware of our position, with lights and strobes!!?) another case of see and avoid, especially on weekends around small airports. Recommendations: 1) tower should make it clear to aircraft when 'not in sight'. 2) lyh should have a radar departure to better monitor IFR traffic. 3). We should endeavor to accurately report positions.

Title: MLG ACFT DECLARED VFR 3 MILES SOUTH OF VOR WHEN THEY WERE ACTUALLY 3 MILES SOUTH OF ARPT. ENCOUNTERED SMA IN PATTERN, BOTH TOOK EVASIVE ACTION. NMAC.
Narrative: PASSED LYH VOR, HAD LYNCHBURG ARPT VISUALLY, CLRED VISUAL APCH BY CENTER. F/O REPORTED POSITION AS '5 S OF THE VOR' (MEANT 5 S OF THE ARPT). I DIDN'T CATCH IT. REQUESTED/CLRED TO ENTER LEFT DOWNWIND RWY 21. CAUGHT YELLOW ACFT IN CENTER OF WINDSCREEN, SAME ALT. EXECUTED HARD RIGHT TURN, HE DID THE SAME. MISSED BY 100-200'. TWR SAID THEY DIDN'T EXPECT US TO BE WHERE WE WERE. (PEOPLE ON GND SAW NEAR MISS BUT TWR WAS UNAWARE OF OUR POSITION, WITH LIGHTS AND STROBES!!?) ANOTHER CASE OF SEE AND AVOID, ESPECIALLY ON WEEKENDS AROUND SMALL ARPTS. RECOMMENDATIONS: 1) TWR SHOULD MAKE IT CLEAR TO ACFT WHEN 'NOT IN SIGHT'. 2) LYH SHOULD HAVE A RADAR DEP TO BETTER MONITOR IFR TFC. 3). WE SHOULD ENDEAVOR TO ACCURATELY REPORT POSITIONS.
